A smiling woman looks at a smartphone displaying an app insight about the connection between restorative sleep and glucose levels, alongside a graph tracking sleep and glucose data. Published by HELLO INSIDE, Europe's women-first metabolic health AI platform. This visual demonstrates how their Controlled-by-Design AI provides personalized data insights, like how sleep impacts metabolic health, illustrating why they built health AI differently. Women can leverage AI-powered coaching and continuous glucose monitoring at helloinside.com to understand and improve their metabolic health across every life stage.

Controlled-by-Design AI: Why We Built Health AI Differently

Hello Inside's ControlledbyDesign AI ensures clinical accuracy, helping 80% of members lose weight and 76. 5% improve symptoms.

TL;DR

• Most general health AI isn't safe enough for individual health guidance due to a lack of clinical accuracy and contextual understanding, particularly in women's metabolic health.

• Hello Inside developed ControlledbyDesign AI, a hybrid architecture that deliberately separates expert clinical knowledge from AI personalization.

• This system uses a "Signal Intelligence Library" of 250+ validated metabolic patterns to ensure safety, explainability, and individual relevance.

• Four years of realworld deployment with 10,000+ members show significant health improvements (e.g., 80% weight loss, 76.5% symptom improvement) and zero adverse clinical incidents.

• ControlledbyDesign AI ensures governance, safety, and explainability are built into its architecture, not added as an afterthought.

WHY WOMEN'S METABOLIC HEALTH NEEDED A NEW ARCHITECTURE

Metabolic dysfunction is the upstream driver of most chronic disease. In 2025, the European Atherosclerosis Society found that 58% of European adults already show systemic metabolic disorders. Globally, metabolic syndrome has more than tripled in two decades from 440 million cases in 2000 to over 1.5 billion in 2023.

The evidence for prevention is strong. Structured lifestyle interventions reduce diabetes incidence by 58% nearly twice the effect of metformin. We know what works. The gap is not knowledge. It is behaviour change at scale.

For women, the challenge is sharper. Despite being disproportionately affected by insulin resistance, PCOS, and hormonerelated metabolic disorders, women are consistently underrepresented in clinical research. Recent benchmarks for large language models in women's health reveal failure rates approaching 60%. The AI that is being deployed at scale is least reliable for the people who need it most.

We believed the field needed a different design philosophy. Not a smarter model. A better architecture.

INTRODUCING CONTROLLEDBYDESIGN AI

ControlledbyDesign AI is Hello Inside's approach to deploying health AI that is simultaneously personalised and safe.

The core principle is a deliberate separation: expert clinical knowledge defines what is safe and plausible. AI determines what is optimal for each individual within those boundaries.

This creates a twolayer system.

Layer 1 is expert knowledge as constraint. Clinical reasoning, evidencebased thresholds, contraindication rules, and escalation pathways are encoded as structured, versioncontrolled definitions. They define the space within which AI is permitted to operate. Safety is not a filter applied after the fact. It is architectural.
